By purchasing from Vivace Health, you agree to the terms and conditions below.

1. I agree to be supplied with Cannabinoid therapy (CBD +/-THC) as recommended by my prescribing Doctor as per recommendations by the Therapeutics Good Administration TGA Dept of Health.
2. I am aware that this product is not registered on the Australian Pharmaceutical List (but approved by the TGA for use in insomnia).
3. I am aware of potential adverse effects that may be experienced as a result of taking the product, and that these must be reported to my prescribing Doctor.
4. I am aware that the cost of medicinal cannabis product is solely my responsibility and not available on the PBS. I am aware that medicinal cannabis use may result in dose adjustments of other medications.
5. I am aware all other non-approved cannabis consumption is prohibited.
6. I am aware that I need to return any unused part of the goods to my prescribing doctor or to a pharmacy for destruction.
7. I am not permitted to drive or operate heavy machinery while impaired using THC.
8. I will undergo regular clinical assessment by my prescribing doctor including potential drug interactions.
9. I understand that medication can not be returned for refund once I have received it in my possession.
